Churchville/Florida: January 30, 2019 at age 83. Predeceased by her son, Barry Spotts. She is survived by her loving husband of 64 years, Gerald B. Spotts; daughter, Dr. Sheri Bisby; granddaughters, Geri Anne “Gigi” Spotts, Kathleen “Katie” Symons; grandson, Wilson “Wil” Symons; step-grandson, Christopher “Chris” Arcidiacono.

Hazel was Past President of the Churchville Fire Department Ladies Auxiliary. She was Vice President and Principal of Churchville Fire Equipment Corp. She was an avid doll collector and had over 2000 dolls. She loved her dogs and she loved her family and she will be missed by everyone.

Everyone is invited to attend Hazel’s memorial service on Saturday July 20, 2019 at 10 am at the funeral home, 21 S. Main St. Churchville. Interment will be private. In lieu of flowers, donations may be made to the Churchville Volunteer Fire Department, Inc. or Alzheimer’s Angels 837 N. Main St. Wildwood, FL 34785
